WHAT TO BRING TO CLINIC?

New patients may fill out the forms online or download the forms in pdf files.
Please bring your
1. Driver License / ID,
2. Insurance card,
3. Medication list (or bottles), and
4. Medical records to your appointment.
If you have pre-clinic lab orders, please make sure you complete lab tests at least 3 business days prior to the visit.

FOLLOW UP APPOINTMENT

As Endocrine specialists, we do in-depth lab testing to check hormone levels. It is important for you to follow up to discuss the results in person (or via telemedicine visit if insurance allows).
Follow up interval is made based on mutual agreement and the clinical condition. We are happy to work with you in setting up the follow-up appointment. We offer in-person follow up (as well as the latest TELE-medicine visits if insurance allows).

We have an ever-growing list of the insurances accepted. We accept most of the commercial and medicare advantage plans, traditional medicare, medicare-medicaid, some market place plans, and some medicaid plans.
